I think the first two problems stem from the third, namely, they don't fit your ears.

Yes, that is one issue with these headphones is that they don't seem to fit in everyone's ears. I'm not sure if there's an easy fix for this.

I'm pretty sure some people sell little silicone covers for them, but I can't help but think that just buying headphones that fit would work much better.
